Brightstorm - These are excellent videos from Algebra 1 to Calculus! You can embed these videos right into your website. The videos are separated into the concept, then one or more practice problems with solutions.
Here's an example:

HippoCampus - I love this for not only finding topics my students need help with, but also searching by specific book. You can find algebra and calculus (in English or Spanish), statistics, physics, biology, and more here!
The lessons are very interactive, and they have exercises at the end of each section. I have had my students write down the problems from these exercises and use them as assignments.

MathTV - You can find videos here that teach basic math, algebra, geometry, trigonometry, and calculus. For most of the videos, you can choose from different instructors (one of them speaks Spanish), and many of them have accompanying worksheets too. These videos can no longer be embedded into your site, but you can share a link to them.

Khan Academy - Wow ... this guy knows everything! These YouTube videos will help your students learn everything from developmental math to AP Calculus BC ... not to mention the science, economics and history.

RobbWorld - a great YouTube channel for Algebra 1, Algebra 2, and AP Calculus AB

If you want to embed any of these YouTube videos into your site, and your school blocks YouTube, you can use Zamzar to download the video, then just add the video as a file.
